Privacy Policy

This Privacy Policy ("Policy") describes how Form Focus LLC ("Company," "we," "us," or "our") handles information through https://formfocusllc.com (the "Website"), including its forms, analytics, security, and support features. It does not govern the Form Focus Android application, which is covered by the separate Mobile App Privacy Policy available on this Website's Privacy page.

California Consumer Privacy Act and California Privacy Rights Act

If you are a California resident, you have the following rights under the California Consumer Privacy Act ("CCPA") and California Privacy Rights Act ("CPRA"):

  • Right to Know. You may request details on what personal data we collect, use, and share.
  • Right to Delete. You can request deletion of personal data, subject to certain legal exceptions.
  • Right to Correct. You may request corrections to inaccurate personal information.
  • Right to Opt-Out. You can opt out of the sale or sharing of personal data for advertising.
  • Right to Restrict Sensitive Data Use. You may limit the use of sensitive personal information.
  • Right Against Retaliation. The Company will not discriminate against you for exercising your rights.

Collection of your Personal Information

We collect personal information when you choose to submit a form on this website. Depending on the form, this may include your name, email address, device information, feedback, request category, request details, document context, and consent or acknowledgment choices. The legal contact form sends this information through a protected Firebase callable function and stores it in Google Firebase Firestore. We use form information to respond to the request you made, verify a privacy request when necessary, provide support, maintain security, and meet legal obligations. We retain submitted form records only for as long as reasonably necessary for those purposes, dispute resolution, and required recordkeeping.

If you affirmatively allow optional site analytics, we collect pseudonymous website-use information through Google Analytics for Firebase. This includes page views, approximate location, device and browser category, referring site, standard UTM campaign identifiers, interactions with important page sections, calls to action and demo videos, form funnel status, and successful lead events. Analytics events do not include names, email addresses, phone numbers, form answers, free-form text, or other submitted form content. With consent, we also store the first campaign, referring page, landing path, and capture time in your browser and attach that attribution to a successful waitlist, beta, or newsletter record in Firestore. We use this information to understand acquisition, improve the website, and measure aggregate visitor-to-lead conversion.

Cookies and Similar Technologies

This website does not use advertising cookies, remarketing, or cross-site advertising tracking, and we do not sell browsing data. Optional Google Analytics remains off until you affirmatively allow it. Our use of storage on your device includes:

  • Theme preference. Your choice of the light or dark version of the site is stored in your browser's local storage under the key theme. It stays on your device and is never sent to us. Clearing your browser's site data removes it.
  • Analytics choice. The first-party local-storage value form_focus_analytics_consent_v1 remembers whether you allowed or declined optional Analytics. It contains only that choice.
  • First-touch attribution. After you allow Analytics, form_focus_first_touch_attribution_v1 stores the original campaign identifiers, referring page without its query string, landing path, and capture time. It is removed if you decline Analytics later.
  • Google Analytics. After you allow Analytics, Google may set _ga and _ga_<measurement-id> cookies for pseudonymous browser and session measurement. These may last up to two years, subject to browser limits or earlier withdrawal. We use Analytics for aggregate traffic, acquisition, engagement, funnel, and lead-conversion reporting, not advertising, personalized ads, Google Ads audiences, or remarketing.
  • Google reCAPTCHA. Our forms are protected from spam and abuse by Google reCAPTCHA v3, which we use through Firebase App Check. reCAPTCHA may load when you open a main form, begin using the secure legal request form, submit a form, or when the site reports a technical error; it does not load simply because you read a page. When it runs, Google sets the _GRECAPTCHA cookie under its own domain and collects device and browser signals in order to score the request. This processing is governed by the Google Privacy Policy and the Google Terms of Service. We use reCAPTCHA solely to keep our forms usable, never for advertising or profiling.

You can allow, decline, or later withdraw optional Analytics through the site's Analytics Settings action. Declining prevents the Analytics tag from loading and prevents Analytics events. Withdrawal stops future collection, removes saved first-touch attribution, and attempts to remove the site's Google Analytics cookies; it does not automatically erase aggregate data already received by Google. You can also block or delete cookies through your browser settings at any time. If you block Google's domains, our forms will not submit, because reCAPTCHA must run before a submission is accepted.

Do Not Track Signals

Some browsers can send a "Do Not Track" ("DNT") signal. There is no common industry standard for interpreting these signals, and this website does not respond to them differently. We do not track you across third-party websites or over time for advertising, and optional first-party Analytics is controlled by the site's explicit allow or decline choice.

Website Logs and Error Reporting

This website is hosted on Google Firebase, which records standard server request data such as your IP address, the page requested, the time of the request, and your browser's user agent. If a script error occurs while you are using the site, we send a diagnostic report containing the error message, the technical stack trace, the page path without its query string or fragment, your browser's user agent, and the build identifier of the site version you were using. We process these reports in our Firebase project and through Functional Software, Inc. d/b/a Sentry, which groups and alerts us to faults.

When Sentry is configured, it also receives a 10% sample of page-loading and request-timing transactions in production so we can diagnose slow or unreliable pages. Our Sentry configuration excludes visitor identity, cookies, request and response headers and bodies, query strings, form contents, local variables, session replay, logs, metrics, and DOM or console breadcrumbs. Diagnostic payloads are configured not to attach visitor IP addresses. Error reports and sampled performance traces are stored for a limited period and are used only to maintain the Website, not for advertising or profiling.

Sharing Information with Third Parties

The Company does not sell, rent, or lease its customer lists to third parties.

The Company uses Google LLC (Firebase Hosting, Cloud Functions, Firestore, reCAPTCHA, and consent-gated Google Analytics) to host the Website, receive form submissions, protect forms from automated abuse, and measure aggregate Website use after consent. We use Functional Software, Inc. d/b/a Sentry for error monitoring and sampled performance tracing as described above. We may use contact information you submit to respond to your request or provide communications you requested. These providers process information only to supply services to the Company under their applicable contractual and confidentiality obligations.

The Company may disclose your personal information, without notice, if required to do so by law or in the good faith belief that such action is necessary to: (a) conform to the edicts of the law or comply with legal process served on the Company or the site; (b) protect and defend the rights or property of the Company; and/or (c) act under exigent circumstances to protect the personal safety of users of the Company, or the public.

Right to Deletion

Subject to certain exceptions set out below, on receipt of a verifiable request from you, we will:

  • Delete your personal information from our records; and
  • Direct any service providers to delete your personal information from their records.

Under the CCPA and CPRA, you have the right to request that the Company, and any third parties with whom your personal information is sold or shared, delete any personal information that has been collected about you. To exercise your rights, use the secure request form displayed on this Privacy Policy page and select the request that best matches what you need. We may ask for additional information to verify your identity or authority to act for another person.

Please note that we may not be able to comply with requests to delete your personal information if it is necessary to:

  • Complete the transaction for which the personal information was collected, fulfill the terms of a written warranty or product recall conducted in accordance with federal law, and provide a good or service requested by you, or reasonably anticipated within the context of our ongoing business relationship with you, or otherwise perform a contract between you and us;
  • Detect security incidents, protect against malicious, deceptive, fraudulent, or illegal activity; or prosecute those responsible for that activity;
  • Debug to identify and repair errors that impair existing intended functionality;
  • Exercise free speech, ensure the right of another consumer to exercise his or her right of free speech, or exercise another right provided for by law;
  • Comply with the California Electronic Communications Privacy Act;
  • Engage in public or peer-reviewed scientific, historical, or statistical research in the public interest that adheres to all other applicable ethics and privacy laws, when our deletion of the information is likely to render impossible or seriously impair the achievement of such research, provided we have obtained your informed consent;
  • Enable solely internal uses that are reasonably aligned with your expectations based on your relationship with us;
  • Comply with an existing legal obligation; or
  • Otherwise use your personal information, internally, in a lawful manner that is compatible with the context in which you provided the information.
